Why incidents occur
Bad things happen to the best of people. Fallibility is a human trait and we cannot eliminate it. What we can do is prepare for it and be ready to mitigate against it. When a medicines safety incident occurs, there will be a front line person who makes an error. We can classify the error by its nature (see table 1).
