This involves identifying who is responsible for implementing and measuring, monitoring and reviewing these solutions on a long-term basis. It also involves setting target dates for completing and reviewing the process. These plans need to be communicated to those who will be affected, ensuring there is a good understanding of what will happen, why it is happening, when it will happen and who is responsible. This communication will help to minimise the risk of confusion and possible errors.

This process should be documented as part of the risk assessment process so that if people want to seek further improvements to mitigate risk in the future, they do not have to repeat all of the same work. Such documents are essential for audits where independent evaluators may need to assess these processes in order to quality-assure that the shortand long-term activities in a pharmacy are acceptably safe.
