To undertake medicines reconciliation, you should be a registered healthcare professional and have:
- Effective communication skills
- Technical knowledge of processes for managing medicines
- Therapeutic knowledge of medicines use.
The three Cs is a common approach to medicines reconciliation. This involves:
- Collecting
Collect the information about a patient's medicines, for example by speaking to the patient and/ or their carer, looking at their summary care record, phoning the GP or looking at repeat prescription slips from the GP. - Checking
Check that the information is accurate by comparing it with other sources of information and that the medicines and doses prescribed are appropriate. - Communicating
Communicate this with the patient and the patient's multidisciplinary team.
Communication is essential in medicines reconciliation and so thinking about consultation skills with the patient will help to ensure the reconciliation is effective. Building a good rapport with the patient, their family/carers and other members of the multidisciplinary team is an important step as it allows patients to feel comfortable in sharing their experiences with their medicines and helps in obtaining accurate information during medicines reconciliation.
