Focus on factors that contribute to medication errors and make effective and sustainable changes. If possible, learn from others to avoid errors occurring.
Reflection exercise
Think about the last time a patient came back to you to tell you about a mistake. Before you investigated, what was your assumption? How did this affect your response to the patient? Would you give the patient an apology even before you know whether a mistake was made? Would it do any harm if you did?
